Family Managed Care Program
Through the Ontario Family-Managed Home Care program, eligible patients or their substitute decision-makers, receive funding to purchase home care services or employ care providers.
This offers greater flexibility and choice as those approved for Family-Managed Home Care choose their provider, determine how and when the care plan is delivered and handle all related administrative tasks including managing providers, funding and reporting requirements.
Our highly skilled PSWs, DSWs, RNs and RPNs can provide round the clock care for children with complex and non-complex care needs. Our field-staff and client care managers have extensive experience in caring for children with special needs receiving funding from the Family Managed Home Care program and can customize a care plan for your child.
Families must first have an assessment completed to determine their needs, meet the general and specific eligibility requirements for traditional home care and have a plan of service developed by Home and Community Care Support Services.

Personal Support Services
- Provide personal care such as feeding, bathing, toileting, ambulation, etc.
- Assist with child's daily routine
- Accompany child to appointments, out of home activities, etc.

- Assist children with their day-to-day behavioural needs

Nursing
- Infusion and injection services
- Wound care
- Monitor child's condition
- Administer tube feedings, medications, etc.
- Ostomy and catheter care
- Tracheostomy and mechanical ventilation
- Provide personal care such as feeding, bathing, toileting, ambulation, etc.
- Perform respiratory intervention such as suctioning, oxygen administration, non-invasive mechanical ventilation (BiPap), and chest physiotherapy
- Accompany child to appointments, out of home activities, etc.
